Definitions:-
- SunMed Velocity Sdn Bhd, a company registered in Malaysia under company registration number 199001018477
- SunMed Velocity Cherish Card is issued by SunMed Velocity
- Card Holder means all the holders of a valid SunMed Velocity Cherish Card
The SunMed Velocity Cherish Card is a privilege card which gives Card Holders the benefits of enjoying discounts, privileges, events from time to time offered by SunMed Velocity
The SunMed Velocity Cherish Card is non-transferable and non-assignable and is only for the exclusive use of the Card Holder only
The SunMed Velocity Cherish Card is NOT a point accumulation card. It does not allow or require Card Holder to collect points for gift exchange or similar purposes
The SunMed Velocity Cherish Card is NOT a credit card, debit card or the like
Discount privileges are valid only upon presentation of your SunMed Velocity Cherish Card during registration. No reimbursement or refund on previous or back-dated transactions if card holders fail to present the SunMed Velocity Cherish Card during registration
Discount privileges strictly apply to self-pay cases only
Unless otherwise stated, the card cannot be used in conjunction with other cards and/or other promotions
SunMed Velocity reserves the right to discontinue, modify or add any terms and conditions, benefits or privileges granted by this card as deemed necessary without prior notice
SunMed Velocity is not liable for any loss or damage incurred by the Card Holders due to any unauthorized use of the SunMed Velocity Cherish Card
Any request for replacement of SunMed Velocity Cherish Card for any reason whatsoever including but not limited to loss or damaged cards, shall be subjected to a fee of RM10 per card
Should any of the conditions and privileges are misused or abused, SunMed Velocity is entitled to suspend and terminate the Card Holder’s right to use the SunMed Velocity Cherish Card without prior notification
SunMed Velocity undertakes that all personal information supplied by the Card Holder for the registration of the SunMed Velocity Cherish Card shall not be disclosed to any third party unless with the prior written consent of the Card Holders or required by law
The SunMed Velocity Cherish Card is a life time membership programme without a need of renewal
The SunMed Velocity Cherish Card is applicable for Malaysian and expatriates aged 60 years and above
The discounts offered for SunMed@Home are only valid on non-packages services. It is also valid on medical supply and mileage charges.
The discount from Sunway TCM Centre is not applicable for consultation, medication and consumables items.
